The archive kept only this opening line, so the terms themselves — rates, deposit, notice period, late fees, termination — are gone. This one is not a form to fill in, it is the contract, so nothing about it gets drafted on this end. Ask her for the current signed version, add it to the list above as a download, and have whoever handles her contracts read it before it goes on a public website.
